Kaitlin Galanos from Unite Us will share information about the Unite California Network which serves San Francisco and San Mateo County in partnership with Kaiser Permanente. The Network connects community partners providing social services with health care providers to deliver integrated whole person care. During this session, you will learn more about the Unite Us technology platform and Network, understand the steps to participate, and be able to ask questions regarding the platform or partnership.
